Lightyear Health is looking for an empathetic and tech savvy Nurse Practitioner to become part of a growing company that improves the functional independence and quality of life of the geriatric and disabled populations. Here’s what you’ll do:

  • Conduct in person visits with patients in skilled nursing facilities.
  • Work with Physical Medicine and Rehab physicians to develop care plans and treatment recommendations for pain management and the improvement of functional and behavioral health in the geriatric populations.
  • Attend weekly team meetings at skilled nursing facilities to help coordinate care and make treatment recommendations.
  • Perform joint injections and botulinum toxin injections for spasticity as needed.

We’d love to hear from you if you have the following:

  • Empathy and a genuine interest in providing care for a geriatric population
  • APRN Texas license. New grads and experienced providers both welcome.
  • Strong interpersonal skills as you will interact with many team members at the company and partnering skilled nursing facilities.
  • Bonus points:
    • Post-acute care experience either in a nursing home or acute rehab setting.
    • Specialty certification in Gerontology

Anticipated Salary Range

We’re committed to an inclusive, consistent, and equitable approach to compensation and anticipate that this position will earn between $110,000 and $150,000 annually. This does not include a stock option grant, which all of our team members receive. The exact salary will depend on (1) how many years of relevant and transferable experience the candidate brings to the role and (2) the internal level at which the candidate is hired in relation to our career development framework.

Note you may be required to travel to locations up to a 90 minutes away from your location -3x per week. Travel expenses (mileage, fuel, meals, lodging ) and travel time will be reimbursed by the company.

The clinical team at Lightyear Health is responsible for meeting and complying with our credentialing verifications processes, clinical standards, health screenings, and vaccination policies at all times. It is the responsibility of the practitioner to ensure that your licenses, certifications, and credentials are kept up to date and in good standing.
