Lightyear Health is looking for a part-time (8 hours, once a week during the weekday)  tech-savvy Care Coordinator to provide assistance during telehealth visits with our clinical team for our geriatric patients and their families. This role offers an opportunity to be an integral team member in a rapidly growing company.

Here’s what you’ll do:

  • Conduct mental health and cognitive assessments such as depression and anxiety screenings and basic memory testing, obtain accurate vitals, know how to populate certain sections of charts such as problem lists and medications.
  • Troubleshoot any technical issues, including a basic understanding of Apple products and software applications
  • Identify communication issues and work with patients and providers if rescheduling is needed
  • Communicate and collaborate with the clinical team

We’d love to hear from you if you have the following:

  • California CNA, MA, EMT, HHA certification preferred
  • Interest in working with individuals with possible dementia and/or mental health diagnoses
  • At least 1 year of experience working in the healthcare industry
  • Strong communicator with the ability to engage with various stakeholders in a professional and warm manner
  • Empathy and a genuine interest in providing care for the geriatric population

*This role requires travel to and from multiple facilities in the area and so demonstrated safety while driving is important to us. The company will reimburse for mileage to and from our facilities*

The clinical team at Lightyear Health is responsible for meeting and complying with our credentialing verifications processes, clinical standards, health screenings, and vaccination policies at all times

Anticipated Salary Range

We’re committed to an inclusive, consistent, and equitable approach to compensation and anticipate that this position will earn $22 per hour. The exact compensation will depend on (1) how many years of relevant and transferable experience the candidate brings to the role and (2) the internal level at which the candidate is hired in relation to our career development framework. All team members also receive a stock option grant.
