Lightyear Health is looking for an on-call Mobile Medical Support professional to facilitate Telehealth Visits with our patients in order to provide world-class care.

You will be called upon to be Lightyear Health’s representative on the ground at Skilled Nursing Facilities in the Stockton, CA region. You will have the opportunity to be a critical member of our best-in-class clinical team while also having the flexibility to choose your days and shifts.

Using our proprietary technology and expertise in complex care, our team collaborates with individuals, families, and caregivers to design personalized care plans that address the holistic needs of each patient.

Key Outcomes:

• On an as-needed basis, facilitate 10-15 telehealth visits daily using a provided iPad 
• Leverage our EMR system for patient registration in real-time
• Be a liaison between Lightyear Health’s patients and our remote clinicians
• Own all aspects of patient check-in and check-out with a diverse population   
• Be responsible for the transport, sanitation, and initiation of telehealth equipment 

The most important skills for the role are*:

• At least 1 year of experience in one of the following role types: Medical Assistant, Medical Secretary, Medical Receptionist, Nursing Assistant, Nursing Aide, Patient Navigator, Patient Care Aide, Patient Care Assistant, Patient Care Technician, Home Health Aide
• At least 1 year of experience working in a medical setting (e.g. you’ve supported clinicians, interacted with patients and have used an EMR before)
• Ability to travel between multiple locations
• Available 2-5 days a week for 4 hours minimum per day

This role requires travel to and from multiple facilities in the area and so demonstrated safety while driving is important to us. The company will reimburse for mileage to and from our facilities.

Compensation: 

We’re committed to an inclusive, consistent, and equitable approach to compensation and anticipate that this position will earn $27 per hour. After 40 hours of work, you will also receive a $1,000 sign-on bonus and a $500 bonus for every referral that is hired by Lightyear Health. The exact salary will depend on (1) how many years of relevant and transferable experience you bring to the role and (2) the internal level at which you are  hired in relation to our career development framework. All full time team members also receive meaningful equity in the form of a stock option grant.
