The clinical team has the privilege of delivering patient care through access established by our sales team, using tools designed by the engineering team, and through best practices informed by our data team.

Clinical team members are dedicated to providing individualized care while combating systemic issues like fragmentation and inaccessibility that disproportionately impact persons in congregate communities.

Clinical Team Roles: Certified Nursing Assistants (CNA), Medical Assistants (MA), Nurse Practitioners (NP), Physician Assistant, Physician Associate, Advanced Practice Provider (APP), Psychiatric Mental Health Nurse Practitioners (PMHNP), Social Workers (LCSW), Geriatric Psychiatrist.
